In order to provide you with the Services, we may collect the following information:
Browser Data - Like most websites, we may collect information about your browser and your use of the Site. This information includes your Internet Protocol address, the type of browser you use, the language setting of your browser, the links you click on within the Site, the link you clicked on to arrive to the Site, the time and date of your visits and clicks on the Site (collectively, "Clickstream Data"). If you log into the Site using a valid user name and password, the Clickstream Data we may collect is associated directly with your login and any cookies we place on your computer. If you do not log in to the Site, such Clickstream Data is collected in an anonymous manner, meaning that we do not ordinarily associate such data with you personally except in the rare event that you are somehow specifically identified by your Internet Protocol address or by the link you clicked on to arrive to the Site.
Log In Information - We may collect personal information necessary for the creation of your account on the Site, including your name, address, telephone number, fax number, email address, place of employment, title as well as the user name and password you select or that is assigned to you ("Login Information").
Email Communications - If you send us emails, we may collect the information in such emails.
Financial Information - We may collect financial information such as credit card numbers ("Financial Information") from individuals designated by Members and Subscribing Systems to transmit payment to HASC for the Services provided by the Site. When we receive Financial Information from you, we print out such information and retain it in our files. We then redact any credit card numbers stored on the Site leaving only the last four digits of the credit card number.
Compensation Surveys - In order to provide Members and Subscribing Systems with our Compensation Surveys, we may collect certain information provided to us by Members and Subscribing Systems. HASC provides Members and Subscribing Systems with questionnaires concerning compensation data for executives, management and non-management employees and officers of hospitals and other healthcare systems. Such data includes average annual salary, salary range, annual bonus, total cash compensation, incentive or bonus plan structure, alternative rewards programs, base hourly rates, salary range, per diem rates, specialty pay, weekend and shift differentials, signing, referral and relocation bonuses and other types of compensation data. We also collect vacancy and job availability data for various hospitals and healthcare systems such as number of employees, part time, full time, number of vacancies and job openings. While Members and Subscribing Systems typically provide the foregoing information to us in an anonymous, aggregate format, some of this information may be correlated personally with certain individuals, particularly the executives of individual hospitals and healthcare systems.
Labor Resource Library - In order to provide Members and Subscribing Systems with our Library Reports from our Labor Resource Library, we may collect certain information provided to us by Members and Subscribing Systems. This information includes data concerning labor activity in the healthcare industry of California including summaries of recent labor activity, profiles of the most active unions (including current leadership structure and union financial reports), a database of NLRB petition filings for union certification and decertification, a database of California hospitals' collective bargaining agreement expiration dates and archives concerning union contracts with hospitals, union financial and organizational information filings and constitutions and bylaws of labor unions and union locals. Much of this information in these Library Reports is collected from publicly available sources but it can contain personal information about individuals.
Web-based Polls - We may collect information from polls posted from time to time on the Site if you respond to such polls.
HASC uses Login Information to create your account, verify that you are affiliated with a Member or Subscribing System, identify you as the proper owner of your account and for communication between HASC and Members or Subscribing Systems concerning such entities' accounts with HASC if you are appointed by a Member or Subscribing System as the contact person for such entity. We do not share Login Information with third parties except if necessary for such third parties assist us in the operation, maintenance and development of the Site. We keep records of all Login Information indefinitely.
If you send us an email, we may use any information we collect from such email to improve the Site or respond to your inquiries. We maintain copies of emails we receive indefinitely.
In the event that HASC undergoes a merger or acquisition with or any substantial transfer of assets to a third party, we may disclose or assign all the information we collect, or have collected, to the merging or acquiring party.
You may update or revise your Login Information or Contact Information by updating your User Profile.
In the future, we may in our sole discretion give you the opportunity to opt-out of receiving communications from us. Until such time, we reserve the right to send you emails or otherwise communicate with you using your Login Information. Such communications could include offers for new or existing Services or other forms of solicitation, news and updates concerning HASC and the Site, invitations to our webinars and other communications. If you are a Labor Activity Specialist, a Compensation Survey Coordinator or if you are designated by a Member or Subscribing System as a contact person for such entity, we may send you reminders concerning the expiration of your subscriptions or communicate with you concerning labor activity and the gathering of information for our Compensation Surveys. Although at this time we do not plan to contact you concerning third party products and services, we reserve the right to do so in the future.
We will use reasonable physical and reasonable electronic security measures to protect your Login Information and Financial Information. HASC limits access to such information to those employees who need to know such information or as otherwise necessary in the reasonable and ordinary course of business. Of course no security system, physical or electronic, is foolproof. While we will make reasonable efforts to protect the information you give to us, we cannot guarantee the safety of such information.
